Here are 4 tips that might solve the problem, if they aren't working you will have to check out the options that you've got left.

1) Restart your console
2) Check for any loose cables
3) Unplug all cables, and plug them back in but don't plug out the power cord.
4) Take out the hard-drive, and get it back in.
